Coordinated Entry
An important process for people experiencing or at risk of homelessness
Coordinated entry is a crisis response system that streamlines services. Individuals’ strengths and needs are quickly assessed. We then quickly connect people to appropriate, tailored housing and mainstream services within Waukesha County.
Standardized assessment tools and practices consider the unique needs of youth, children, and families. This assessment allows households to gain access to the best options to address their needs, incorporating participants’ choice, rather than being evaluated for a single program within the system. The most intensive interventions are prioritized for those with the highest needs.
Recovery Voucher Program
There are many reasons for homelessness, and substance abuse is just one of them. For those experiencing homelessness and opioid addiction, we can help with a grant that provides financial assistance for housing.

Youth System Navigator
The Youth System Navigator serves as a “point person” in Waukesha County for youth ages 18-24 experiencing homelessness and crisis situations. The navigator provides referrals to community-based resources and provides guidance so that further crisis can be avoided. The Youth System Navigator will meet with each participant one-on-one on an ongoing basis. With awareness of each person’s schedule, needs, interests, and strengths, they collaboratively develop individual plans and continually monitor its implementation. Additionally, the Youth System Navigator conducts community outreach and advocacy.

Permanent Supportive Housing
Permanent Housing assists low-income households with the support they need to live independently. Residents receive supportive services to develop a housing stability plan to address their housing issues.
